Abstract

The analysis of primary disablement in children due to eye diseases including lens pathology was conducted as well as determination of lens pathology incidence in the nosologic structure of children's disablement due to eye diseases in the Republic of Bashkortostan (RB) according to the Chief Bureau of Medical Social Examination in the RB. Introduction of the developed system of rehabilitation allowed significantly decreasing the primary disablement in children in the RB during 10 years.

Potentials of contact correction of vision in premature infants with abnormal refraction

Abstract

Clinical follow-up in dynamics at the age from 5 to 10 were conducted in 62 myopic children born premature. The patients were divided into 3 groups: 16 children with mild myopia, 18 children with moderate myopia and 28 children with high myopia. The second part of the study included of 63 children (77 eyes) after lensvitrectomy followed-up during 10 years. The authors proved that early adequate contact correction is indicated to the children with highly abnormal refraction for correct and full development of visual analyzer. Increase of visual acuity as well as that of visocontrastometry indices was noted in patients with moderate and high myopia in contact lens wear in comparison with glasses correction. In patients after lensvitrectomy performed in IV-V stage of retinopathy of prematurity in 82% of cases the difference in child's behavior in contact lenses wear and without it was noted. Deviation and amplitude of nystagmus diminished in contact lenses wear. According to electrophysiological indices increase of evoked visual potentials and decrease of latency were noted in these children.

Features of refractogenesis in infants with retinopathy of prematurity during initial years of life

Abstract

The article presents the analysis of structure and dynamics of refraction in 147 children (238 eyes) with retinopathy of prematurity of I-III stage during initial years of life under the type of "longitudinal section" at 6, 12, 18 and 24 months of life. It was detected that during all the studied age periods myopia was the prevailing type of abnormal refraction in children with retinopathy of prematurity and its incidence decreased from 73.7% at 6 months to 60.5% at 24 months. Incidence of myopia including high myopia was significantly higher in the group of induced regression in comparison with the group of spontaneous regression (76 and 28.2% respectively). In children with retinopathy of prematurity instability of refraction during the initial years of life is a feature of refractogenesis, which may change both towards amplification and attenuation of refraction. Myopia formation in premature infants occurs against the background of eyeball growth retardation after premature birth; at that the lens and the cornea play the leading role in myopia development.

Features of myopia progressing depending on the intraocular pressure level

Abstract

The intraocular pressure and hydrodynamic indices were studied in 190 infants and adolescents (375 eyes) with progressive myopia, 30 infants (60 eyes) with stationary myopia and 30 infants (60 eyes) with emmetropia. Hydrodynamic indices were studied depending on intraocular pressure values. The analysis of the obtained data allowed arriving at the following conclusions: intraocular pressure in patients with progressive myopia was higher (16.39 mm Hg) than in emmetropic patients (14.84 mm Hg) and in patients with stationary myopia (14.69 mm Hg). Regarding this index in the group with stationary myopia and emmetropia no significant differences were detected. Myopia progressing may be accompanied by high IOP norm (31.8% of cases), moderate IOP norm (48.8%), and low IOP norm (19.4%). In these subgroups we could distinguish patients with reduced coefficient of intraocular fluid outflow and its relative hypersecretion. A comparative evaluation of changes of ocular axial length in convergence and after visual load (40 minutes of reading) was conducted. Reasoning from these results of the examination it was possible to suppose that in myopia progressing accompanied by low IOP norm the mechanics of eye elongation in convergence under the influence of extraocular muscles is in existence.

Accommodative asthenopia in schoolchildren with mild and moderate ametropia

Abstract

The authors followed up 40 schoolchildren with accommodative asthenopia at the age from 8 to 17. The patients were divided into 2 groups according to the refraction type: with mild and moderate myopia (mean -1.93+0.18 D) and with mild and moderate hyperopia (mean +2.08+0.28 D). Weakness of accommodation in the nearest zone of clear vision and persistent strain of accommodation (mean 1.0 D) in the further zone of clear vision was noted in schoolchildren both with myopic and hyperopic refraction due to the overstrain of visual system. In accommodative asthenopia nonsynchronous strain of both eyes accommodation was present being expressed in anisometropic accommodation in myopia in the nearest zone of clear vision (mean 1.52+0.19 D), and in hyperopia both in the nearest and in further zone of clear vision (mean 2.43+1.16 D). Normalization of indices of absolute accommodation in patients with myopia and hyperopia leads to the improvement of the vision at far and at near and to asthenopia elimination in 95% of cases.

Distraction scleroplasty in surgical treatment of progressive myopia in children

Abstract

Method of surgical treatment for progressive myopia in children was developed that allowed changing of the direction of tensile stress in equatorial area of myopic eyes sclera. The developed surgical technique for progressive myopia treatment is highly efficient and technologically easy, the material convenient for implantation is available and complications are practically completely absent. Favourable influence of mechanical induction artificially created by distraction of the sclera and stimulating reparative regeneration maintains the level of compensatory-adaptive processes in the sclera of the eye for a long period. Distraction scleroplasty in the authors' opinion is a promising method of surgical treatment for progressive myopia in children with anterior-posterior axis of the eye up to 26.0 mm.

Experience of "Chondroitin-AKOS" drug application in progressive myopia treatment in children

Abstract

Progressive myopia remains a topical problem of modern ophthalmology. The article in question presents the dynamics of visual functions indices and connective tissue metabolism in treatment by "Chondroitin-AKOS" drug in 40 children with progressive moderate and high myopia. It was detected as follows: visual acuity improvement, increase of volume and resource of relative accommodation, increase of type I carboxi-propeptide-procollagen level in blood, decrease of the oligomerous matrix protein of cartilage and cartilaginous glycoprotein-39. The analysis of visual functions, anterior-posterior size value of the eye 6 months later has shown that medical effect of the drug still remained.

Abstract

Contrastive analysis of efficiency of progressive axial myopia treatment by medical and surgical methods was conducted. It was detected that these methods could be considered as equivalent at a moderate pace of myopia progressing. It is necessary to treat progressive myopia at an early age in the presence of corresponding indications as at the age over 13 spontaneous stabilization of the process occurs in non-complicated myopia.

Analysis of complications incidence and structure in the long-term period after congenital cataract aspiration with IOL implantation in children at different age of early childhood period

Abstract

The data of study of the incidence and structure of complications in the long-term period after congenital cataract aspiration with IOL implantation in children at different age of early childhood are presented. The results were analyzed in 64 children (92 eyes) at the terms 1.5-3.5 years after the operation on reaching the age of 4 by the patients. The obtained data indicated that the secondary cataract was the main post-op complication. Fibrosis of posterior capsule of the lens was the most frequent (53.0+1.8%) type of the secondary cataract that most often developed in children at infancy and in the initial period of the early childhood during initial 1.5-2 months, and in children at the final period of the early childhood in 4-5 months after surgery. The incidence of post-op inflammatory reactions was not high making 7.2+0.5% of cases among the infants of all ages.

Method of non-invasive treatment for terminal glaucoma in children with congenital aniridia and buphthalmos

Abstract

A new method of non-invasive palliative treatment in terminal glaucoma stages in children taking into account the peculiarities of the eyes with congenital aniridia and buphthalmos (patent of the Russian Federation for invention ь2220691) was suggested. The method constitutes a diode laser trans-scleral cyclocoagulation (device OcuLight SLx; infrared spectrum, 810 ?m) with circulatory applications made at the ciliary body projection zone determined by means of diaphanoscopy except the zones of staphylomas and preceding surgery. Laser power mode depended on thickness of the sclera and the adherent tunics in the zone of future impact determined by ultrasonic B-scanning. Hypotensive effect was obtained in 84.6% of operations and remained in the long-term periods (3.9+0.4 years) in 61.5% of cases. The application of the aforesaid method allowed salvaging the eye as an organ without eyeball lancing with good cosmetic effect, at that in 84.6% of cases without anesthesia.

Long-term results of congenital glaucoma surgical treatment

Abstract

The article presents comparative results of external trabeculotomy and sinusotrabeculotomy in children with congenital glaucoma at the aspect of age. The analysis of the obtained data has shown that at the age under 1 year hypotensive efficiency of both operations was approximately equal but there were much more complications after sinusotrabeculotomy. In the groups of patients of the older age the efficiency of external trabeculotomy decreased whereas that of sinusotrabeculotomy remained at the same level. The conclusion was that at infancy trabeculotomy ab externo should be preferred.

Abstract

The results of histological study of the rabbit's eye structures after impact of LED blue laser "AST" of various light powers are presented. It was detected that the dose of laser radiation under 150 J/cm2 has no damaging effect on the ocular tissues. Starting from the dose of light of 300 J/cm2 tissues morphology and eyeball structures changes of various intensiveness were noted.

Abstract

The article described a patient with bilateral lesion of the occipital lobe in the area of calcarine sulcus. The highly informative qualities and precision of modern methods of examination such as perimetry, electrophysiologic examination, optical coherent tomography, computed and magnetic resonance tomography were shown. Thanks to these methods the correct stage-by-stage diagnostics of the level of visual analyzer lesion is possible. Probable nosologic forms in the described symptomatology (Minamata's disease, hemolytic ovalocytic anemia) were analyzed.
