Evaluation of visual performance depending on tolerability of optical correction
Abstract
Accommodative asthenopia is common in myopic patients when permanent full correction is prescribed, since their accommodative system does not tolerate increased stress due to close work.
AIM: To evaluate visual performance in myopic school children depending on tolerability of optical correction.
MATERIAL AND METHODS: A total of 154 children (11 to 16 years) with different levels of myopia, wearing corrective spectacles. Of them, 56 and 98 school children had good and poor tolerability of optical correction according to computed accommodation test, respectively. Accommodation was measured using autorefractor Righton Speedy-K ver. MF-1. Visual performance was evaluated using Bourdon test. Performance accuracy (At), accuracy factor (Ta), productivity (Pt), endurance factor (Kp), and speed of information processing (S) were determined.
RESULTS: In the group where spectacle correction caused asthenopia during visual tasking, performance accuracy, productivity, and accuracy factor exceeded the mean values in children without signs of accommodative asthenopia. But endurance factor and speed of information processing are significantly lower compared to the children with good tolerability of optical correction.
CONCLUSION: Tolerability of optical correction affects visual performance of school children. Poor tolerability of optical correction significantly decreases visual performance parameters, such as performance accuracy, productivity, and endurance.



Evaluation of activity and severity of uveitis in children based on comprehensive biochemistry analysis
Abstract
AIM: To investigate the role of proteolytic enzymes and their inhibitors in endogenous uveitis in children and develop new diagnostic criteria.
MATERIAL AND METHODS: A total of 76 children (135 eyes) aged 3 to 17 years with uveitis were examined. All children underwent standard ophthalmological examination, posterior segment optical coherence tomography (OCT) was performed if required. Location, course, and activity of uveitis were assessed as per criteria developed by the International Uveitis Study Group. All patients underwent tear and serum biochemistry analysis, aqueous humor was tested if surgical treatment was required. Alpha-2-macroglobulin (α2-MG), matrix metalloproteinase-9 (MMP-9), tissue inhibitor of metalloproteinases 1 (TIMP-1), urokinase-type plasminogen activator (uPA), and plasminogen (PG) levels were determined. Concentrations of α2-MG were measured by an enzymatic method using a specific substrate Nα-benzoyl-DL-arginine 4-nitroanilide (DL-BAPNA). MMP-9, TIMP-1, uPA, and PG levels were determined using an enzyme linked immunosorbent assay (ELISA). A total of 64 children (84.2%) were followed-up. The analysis included 1143 tear, 283 serum, and 79 aqueous humor samples.
RESULTS: Active uveitis was found to be associated with activation of proteolytic enzymes and their inhibitors resulting in increased tear uPA levels, as well as increased uPA, MMP-9, and TIMP-1 levels in aqueous humor. Tear uPA level exceeds 80.7 pg/mL in active uveitis, ranges from 46.6 to 80.6 pg/mL in moderate/subactive disease, is less than or equal to 46.5 pg/mL in inactive phase/remission; it is one of the objective criteria to determine uveitis activity. In patients with panuveitis, tear MMP-9, PG, and α2-MG levels were higher compared to the children with anterior and peripheral uveitis.
CONCLUSION: New data on the role of the proteolysis system in the pathogenesis of endogenous uveitis in children have been obtained. New criteria for assessing uveitis activity and severity have been established based on proteolytic enzymes and their inhibitors levels in tear fluid. The obtained results can be used to develop targeted therapy.



Comparative evaluation of choroidal thickness in children using different myopia correction methods
Abstract
AIM: To measure choroidal thickness in the cross-sectional images of myopic patients using different correction methods.
MATERIAL AND METHODS: Examination included 162 patients (324 eyes) aged 9 to 19 years (mean: 11.6 ± 2.7 years), with myopia between 0.5 D to 11.75 D (mean: 4.0 ± 2.3 D) using different myopia correction methods, e.g., monofocal spectacles (MF): 96 eyes, Group 1), perifocal spectacles (40 eyes, Group 2), orthokeratology lenses (OKL) (36 eyes, group 3), bifocal soft contact lenses (BSCL) (86 eyes, Group 4), and Stellest® spectacles (66 eyes, Group 5). The anterior-posterior axis and subfoveal choroidal thickness (non-cycloplegic), and cycloplegic autorefraction were measured in all patients.
RESULTS: In general, choroidal thickness (CT) and CT to the anterior-posterior axis (CT/APA) ratios were 328.6 ± 15.6 μm and 12.9 ± 0.6 in OKL wearers, 298.8 ± 9.4 μm and 12.2 ± 0.4 in Stellest® wearers, 267.4 ± 8.3 μm and 10.8 ± 0.3 in BSCL wearers, and 264.6 ± 12.4 μm and 10.4 ± 0.5 in perifocal spectacles wearers, respectively. The lowest CT and CT/APA ratios were in the monofocal group (241.9 ± 5.3 μm and 9.5 ± 0.2, respectively). The difference in these parameters between the monofocal and other groups was statistically significant (р < 0.01).
CONCLUSION: This study of the cross-sectional images convincingly demonstrates significantly greater choroidal thickness in myopic patients using optical correction methods imposing myopic defocus on the retinal periphery, compared to monofocal spectacles.



Correction of peripheral myopic defocus with HAL spectacle lenses
Abstract
Current methods to slow myopia progression based on the theory of peripheral defocus have shown their efficacy when used as spectacle, contact, and orthokeratology lenses. Spectacle lenses with highly aspherical microlenslets (Stellest®) were introduced into clinical practice in 2020, and their efficacy was rated highly in different studies.
AIM: To investigate peripheral defocus imposed by Stellest® spectacle lenses in myopic children.
MATERIAL AND METHODS: Peripheral refraction (PR) was evaluated in 42 children (84 eyes) with low-to-moderate myopia. Patients of Group 1 (42 eyes) were examined under cycloplegic conditions, without correction and with HAL spectacle lenses, in the primary position and different directions of gaze, 15° and 30° temporally (T) and nasally (N) from the fovea. Patients of Group 2 (42 eyes) were examined under mydriatic conditions, without correction and with HAL spectacle lenses, 5°, 10°, 15° nasally and temporally from the fovea, in the different directions of gaze. PR was measured using the Grand Seiko WAM-5500 open-field binocular autorefractor. To calculate peripheral defocus, central (axial) refraction was subtracted from the peripheral spherical equivalent taking into account the +/− sign.
RESULTS: HAL spectacle lenses reduced hyperopic defocus and imposed a myopic one in all tested areas of the near retinal periphery; the differences at N5 and N10 points were statistically significant (р < 0.05). At N15 point ocular movements imposed myopic defocus of −0.26 D (р < 0.05). There is also a trend towards a decrease in hyperopic defocus at T15 and N30 points.
CONCLUSION: The first study of peripheral refraction with HAL spectacle lenses (Stellest®) helped demonstrate that the lenses imposed myopic defocus on the retinal periphery, with the greatest defocus on the near nasal periphery.

Ophthalmic features of mucolipidosis type I (sialidosis): a clinical case. Ophthalmology aspects for neurologists and pediatricians
Abstract
A very rare clinical case of a juvenile form of a storage disease mucolipidosis type I (sialidosis) is presented. Ophthalmic features include a bilateral macular cherry-red spot. Bilateral macular optical coherence tomography (OCT) revealed hyper-reflectivity of the ganglion cell layer.
CONCLUSION: A cherry-red spot is specific not only for central retinal artery occlusion but also for storage diseases, such as gangliosidoses (Tay-Sachs disease, Sandhoff disease, etc.), mucolipidoses, etc. Ophthalmological examination may be the only key to identify serious systemic diseases, and timely genetic testing might be crucial for a child to determine the adequate therapy. This case was characterized by a typical ophthalmic presentation of sialidosis type I with unclear neurological symptoms suggestive of Tay-Sachs disease. Ophthalmological examination revealed a cherry-red spot with a slow progressing decrease in best-corrected visual acuity (BCVA) which is typical for sialidosis type I but not for Tay-Sachs disease. A neurologist observed the symptoms more characteristic of Tay-Sachs disease than sialidosis type I; they included unsteady gait, ataxia, and dysarthria. There was no myoclonic activity characteristic of sialidosis type I. Thus, genetic testing to identify NEU1 mutations was the only method to objectively examine the patient and determine possible supportive therapy.



Anterior diffuse scleritis, map-like corneal ulcer, and hypopyon anterior uveitis associated with herpes zoster
Abstract
AIM: To analyze etiopathogenesis, clinical features, and a treatment algorithm for acute anterior diffuse scleritis, map-like corneal ulcer, and hypopyon anterior uveitis in order to increase medical awareness of herpes zoster in children.
RESULTS: Long-term exposure to rapid temperature changes contributed to varicella-zoster virus (VZV) reactivation in a child leading to herpes zoster rarely occurring in children, onset of anterior diffuse scleritis and hypopyon anterior uveitis. Corticosteroid therapy without causal treatment led to herpes virus reactivation in the patient’s cornea which contributed to herpes corneal ulcer.
CONCLUSION: Etiopathogenesis features were analyzed, characteristic clinical symptoms of anterior diffuse scleritis, herpes corneal ulcer, and hypopyon anterior uveitis caused by varicella-zoster virus (VZV) reactivation were described. Reactivation led to herpes zoster with skin eruption and herpes zoster ophthalmicus, such as injury of the ophthalmic branch of the trigeminal nerve without extraocular (skin) symptoms of herpes infection. The VZV etiological role was established based on high titers of anti-VZV IgG antibodies, the presence of anti-VZV-gE IgG antibodies (markers of active viral replication), and the efficacy of anti-herpes therapy. The described clinical symptoms of combined ophthalmic pathology contribute to the early diagnosis of ocular herpes for timely antiviral herpes therapy to prevent chronic disease and complications and to preserve and/or recover visual acuity.

X-linked retinitis pigmentosa: clinical manifestations and diagnosis
Abstract
X-linked retinitis pigmentosa (XLRP) is considered one of the most severe forms of retinitis pigmentosa (RP). It accounts for 6–20% of all disease cases. Nowadays, mutations in three genes (RP2, RPGR, and OFD1) have been identified to cause XLRP. Early symptoms are impaired night vision and/or visual field loss. The subsequent damage to the cones results in decreased visual acuity, photophobia and color vision disorders may also be observed. Ophthalmoscopy shows waxy optic disc pallor, vasoconstriction, foci of hypopigmentation and/or hyperpigmentation, sometimes in form of bone-spicule in the peripheral retina. Gradual reduction in amplitudes of light-adapted and dark-adapted electroretinograms (ERG) is specific for RP. Microperimetry reveals reduced average light sensitivity and paracentral ring scotoma despite normal or slightly decreased visual acuity. Optical coherence tomography (OCT) demonstrates destructed outer retinal layers, short-wavelength autofluorescence imaging shows a Robson-Holder ring. X-linked RP in females is almost always associated with notable variability in severity from asymptomatic cases to severe ones as in male patients. Night blindness is reported in 50% of female carriers of X-linked RP, visual acuity and amplitude of light-adapted and dark-adapted ERG are reduced in most cases. Fundus changes are observed in 87% of female carriers. A tapetal reflex is the most common. Foci of hypo- and hyperpigmentation, bone-spicule-like pigment clumping, extensive retinal pigment epithelial and choroidal atrophy are also reported. In the vast majority (63–79%) of female carriers of X-linked RP, short-wavelength autofluorescence images show radial patterns.



Myopia pandemic by 2050: myth or reality? Global strategies to control myopia based on the 19th International Myopia Conference
Abstract
Myopia is the most common cause of impaired distance vision. It can lead to pathological myopia and associated serious complications if progresses to a higher grade. Given the high prevalence of myopia among children and adolescents, it is becoming a medical and social problem. The paper provides a review of lectures and discussions of the 19th International Myopia Conference held in September 2024 in China. Conference papers show that scientists and clinicians are focused on comprehensive investigation of the mechanism of myopia development, identification of its predictors and genetic aspects. A lot of focus is on the elaboration of effective treatment and preventive measures for children of preschool age and older, particularly new pharmacological approaches and improved optical correction methods to control myopia.
