The current stage of the ophthalmological care service in the Russian Federation (2012 - 2013)

This article was designed to report the results of the analysis of the statistical record-keeping forms for the years of 2012 - 2013. It has shown that the prevalence of eye pathology among the children and adolescents in this country is 23% higher than among the adult population. It amounts to 13,167.6 per 100,000 child population and shows the well-apparent tendency toward a further rise (compared with 13,144.1 per 100,000 in 2012). Moreover, the study revealed the incomplete coverage of regular medical examination of the patients presenting with the diseases creating the high risk of disablement, such as glaucoma and retinopathy of prematurity. At the same time, the high medical aid appealability rate was found to be insufficient to ensure the early detection of these conditions and prevent the increase in the number of the neglected cases responsible for the enhanced disability rate. The combined approach to the organization of the ophthalmological treatment of the children and adolescents at all levels of health care service is expected to make it possible to purposefully plan the necessary prophylactic and rehabilitative measures.

The role of neuropeptide therapy in the pediatric ophthalmological practice

We discuss the results of experimental and clinical studies of the drug Cerebrolysin. Neuroprotective and antioxidant effects provide an efficient use of Cerebrolysin in its introduction through an irrigation system in the tenon and retro-bulbar space in children with partial atrophy of the optic nerve of various etiology. Good tolerance and the possibility of combined use with other drugs are the basis for the wide use of the drug in ophthalmic practice in children.

Results of applying of drainage xenoplast in glaucoma surgery in children

The present article reports the results of the analysis of the outcomes of glaucoma surgery by means of anti-glaucomatous trabeculectomy in 10 children at the age varying from 10 months to 15 years (11 eyes) presenting with various forms of this condition (post-traumatic, aphakic, post-keratoplasly, post-uveal, congenital glaucoma) in combination with the implantation of the biological drainage Xenoplast (Russia). The effectiveness of the treatment was estimated from the degree of compensation of intraocular pressure and visual acuity, dynamics of the size of the eye and the morphometric characteristics of the disk of the optic nerve (obtained in the HRT study). Normalization and stabilization of intraocular pressure were achieved in 100% of the cases soon after surgery and in 82.8% of the patients within 1 year after the treatment. No severe postoperatve coplications were documented.

The application of modern fluoroquinilones for the treatment of bacterial eye infections in the young children

The results of the ophthalmological observation of 50 (63.3%) children at the age from 1 to 29 days and 29 (36.7%) ones at the age from 1 to 12 months are reported. 62 (78.5%) patients presented with the signs of bacterial conjunctivitis while the remaining 17 (21.5%) ones suffered from suppurative dacryocystitis. We studied the microflora from the conjunctival space of these 79 children. The Gram-positive bacteria St. epidermidis (59.7%) and St. aureus (21.7%) were found to play the leading role in the development of inflammatory eye diseases. The study of sensitivity and resistance of the antibacterial preparations to the isolated pathogens showed that fluoroquinolones exhibit the highest therapeutic effectiveness. The 0.5% moxifloxacin eye drops caused positive dynamics of the clinical course of inflammatory eye diseases in the newborn and young children. Specifically, inflammation was totally eliminated in the patients presenting with bacterial conjunctivitis and in 71% of those with suppurative dacryocystitis. The authors recommend to use Vigamox eye drops for the wide application in the pediatric ophthalmological practice. However, this preparation can be prescribed to the children under 1 year of age only after obtaining the voluntary informed consent of the parents.

The prognostic and diagnostic significance of the objective accommodation response

The objective of the present study was to evaluate the parameters of accommodation and myopia progression rate depending on the objective accommodation response (OAR) in 130 patients (260 eyes) at the age from 6 to 23 (mean 11.26+-0.2) years presenting with myopia of different severity. All the patients were divided into 3 groups. The first group was comprised of 40 patients (80 eyes) having the objective accommodation response of less than -1.5 diopters. The second group consisted of 63 patients (126 eyes) with the objective accommodation response varying from 1.5 to -2.5 diopters. The third group contained 27 patients (54 eyes) having the objective accommodation response above -2.0 diopters. The patients with the objective accommodation response close to the normal value (>2.0 D) experienced the lowest annual myopia progression gradient during the treatment (0.5D per year retrospectively and 0.17 diopters prospectively) in comparison with the patients of groups 2 and 3 (p <0.05). These findings suggest the relationship between the objective accommodation response and progression of myopia. The results of this study give evidence of the prognostic value of the objective accommodation response and the myopia progression rate and suggest the possibility of using this parameter for the characteristic of the clinical course of myopia. The combined surgical treatment of primary congenital glaucoma in the young children

The present study based at the Department of Ophthalmology, Tashkent Pediatric Medical Institute, was designed to evaluate the antihypertensive effectiveness of the combined surgical treatment of 60 young children (120 eyes) presenting with primary congenital glaucoma using our original surgical procedure. The patients were divided into two groups one of which was comprised of 40 patients (80 eyes) treated by the newly proposed method while the other one contained 20 children (40 eyes) given the conventional surgical treatment who served as controls. The three-year observations have demonstrated that intraocular pressure in 2 (2,5%) eyes of the children in the first group with the advance and terminal stages of the disease tended to increase despite good preservation of the visual function. The repeated operation by the new method was needed within 3 years after primary surgery. Secondary surgery was needed on 5 (12.5%) eyes in the control group within 10 months after the conventional treatment. It is concluded that the new method allows the number of repeated surgical interventions to be reduced by 10%.

The balance of the vegetative nervous system and cardio-respiratory parameteres in children with different clinical refraction in the growth process and its influence on the refraction formation

The study included 155 children (310 eyes) aged from 4 to 18 years. The state of the vegetative nervous system was assessed by Kerdo index in children of different age groups with different refraction. The relationship of vegetative status and cardiorespiratory system indexes was studied. We revealed the predominance of the vegetative nervous system sympathetic component in 88.4 % of children in the main group and in 79.5 % of children in the control group. The effect of parasympathetic vegetative nervous system was detected in 10.1% of children in the control group and in 6.4 % of children in the main group, which indicates their poor physical development. Increased sympathetic tone indicates improvement in basal metabolic rate (hyperkinetic hemodynamics type) by increasing cardiac activity. This research revealed an imbalance of the vegetative nervous system in the modern generation that requires a more objective examination of children and control of their normal development.

A clinical case of the successful treatment of Irvine-Gass syndrome in a child

The clinical manifestations and successful therapy of Irvine-Gass syndrome in a 12 year-old child are described. It was shown that Irvine-Gass syndrome can develop not only in the form of vitreo-corneal traction leading to macular oedema not infrequently with the formation of the cysts in the macula but can also be accompanied by the traction effect on the disk of the optic nerve as revealed by optical coherent tomography (OCT). The use of YAG laser irradiation for the dissection of the vitreo-corneal adhesions can eliminate traction Irvine-Gass syndrome without the surgical opening of the eye and result in the restoration of the normal anatomical structures of the eye with the improvement of visual acuity and the prevention of the development of macular degeneration leading to the irreversible loss of vision in a child.

Prevalence of eye diseases in children in the Azerbaidjan republic

The present analysis of the characteristics of eye diseases in Azerbaidjan (including the absolute number of the patients per 100,000 child population, the prevalence of myopia, blindness, and visual impairment) demonstrates the progressive rise in the prevalence of these conditions in the Republic as a whole and in most of its separate regions. This increase can be attributed in the first place to both the overall tendency toward the growth of eye pathology and to the use of the modern diagnostic tools for the early detection of the eye diseases.

The authors consider the specific features of the organization of the combined (medical, social, pedagogical, public, and psychological) support of the children presenting with retinopathy of prematurity in the Tyumen region. The model for the organization of rehabilitation of such patients is proposed. The high prevalence and social significance of visual disturbances in the children dictate the necessity of the search for the new effective approaches to their prevention and correction.
